About This Program
Through Franciscan Health’s twelve-month Diabetes Prevention Program, patients can expect to take charge of their health with the support and guidance of our certified healthy lifestyle coaches, as well as fellow participants. Our safe environment provides patients with the resources necessary to build energy and confidence, lose weight naturally and reduce stress and anxiety
Program goals include:
- Reduce body weight by 5 to 7 percent
- Increase physical activity to 150 minutes per week
This program follows guidelines recommended by the Center of Disease Control (CDC) that are proven to prevent or delay type 2 diabetes. Sessions are limited.
Individuals who have been diagnosed with Type 1 or Type 2 diabetes do not qualify for this program.
What is prediabetes?
When a person has higher-than-normal blood sugar levels, it means they have prediabetes. Without lifestyle changes, people with prediabetes are at risk of developing type 2 diabetes.
Who is at risk?
Known risk factors include:
- Obesity
- 45 years & older
- Having a parent, brother or sister with type 2 diabetes
- Being physically active less than 3 times a week
- Previously having gestational diabetes or polycystic ovary syndrome
- Of one of the following ethnic groups: African Americans, Hispanic / Latino Americans, American Indians, Pacific Islanders and some Asian Americans are at higher risk
What will insurance cover?
This holistic improvement program is covered by some insurance plans; Medicare coverage is subject to location.
