Online Submission Request

Franciscan Health partners with MRO Corporation to facilitate release of medical and billing records.

Use the Online Request Form to submit a request for your medical and billing records from any Franciscan hospital, provider office or other Franciscan service location.

Please have your government-issued photo ID available.

If you prefer to fill out a printable form manually, use our Printable Request Form to submit a request for your medical records or have copies of your records sent to a third party.

After completing the Printable Request Form, submit it using one of the methods listed below.

Info Needed To Request Medical Records

You will not need any additional verification if you are accessing health records via your MyChart account that is already set-up.

Otherwise, please have your government-issued photo ID available.

If you are a healthcare representative, power of attorney or legal guardian, you will be asked to upload or provide additional documentation to verify that you are legally authorized to make the request on behalf of the patient.
