LAFAYETTE, Indiana - Franciscan Health is pleased to announce the opening of the Lafayette Franciscan Senior Health and Wellness PACE Program. PACE stands for Program of All-Inclusive Care for the Elderly (PACE). Franciscan PACE programs offer comprehensive healthcare for seniors who want to continue living at home. PACE also provides socialization opportunities to help seniors enjoy their freedom and independence. 

PACE healthcare is provided by an interdisciplinary team of caregivers consisting of a primary care provider, registered nurse, social worker, dietitian, physical and occupational therapists and other support staff, all in one location. 

PACE services include:

  • Medical care (physicians and advanced practice providers)
  • Therapies (physical, occupational)
  • Prescription drugs
  • Meals
  • Recreation
  • Personal care
  • Dietician
  • Medical Social Worker
  • Transportation

To qualify for the Franciscan Senior Health and Wellness PACE program in Lafayette, participants must:

  • Be age 55 or older
  • Reside in Tippecanoe County or covered areas of Benton, Carroll, Clinton, Fountain, Montgomery and White counties
  • Have chronic health conditions that qualify for nursing home care
  • Be able to live safely in the community with PACE support

The Lafayette PACE center is officially open to enroll participants. The center is located at 1116 North 16 Street. Other PACE Centers within the Franciscan Health family include Dyer, Indianapolis and Michigan City.
