At Franciscan Senior Health & Wellness, PACE (Program of All-inclusive Care for the Elderly) is a complete healthcare solution for seniors who want to remain at home and live as independently as possible.

PACE (Program of All-inclusive Care for the Elderly)

Our Approach

PACE (Program of All-inclusive Care for the Elderly) is a community-based healthcare program created for people 55 or older who are eligible for nursing-home level care but prefer to receive care in their own familiar surroundings. At Franciscan Senior Health & Wellness, our program enables seniors to live as independently as possible by providing truly all-inclusive elderly care services in their home and community. 

In addition to addressing each participant's unique medical needs, we also help with the social, emotional and practical challenges that older adults often face. Through interactions with peers, stimulating activities and other supportive services, participants will enjoy a better quality of life. 

Franciscan Senior Health & Wellness is a member of the National PACE Association (Program of All-Inclusive Care for the Elderly). The PACE model of care is centered around the belief that the well-being of seniors with chronic care needs, and their families, are better served in their own communities whenever possible.

Complete Senior Health Care

Franciscan Senior Health & Wellness' PACE is a complete health care solution for seniors who need health care services but want to remain at home. This program offers home health care, outpatient senior health services, dental care as well as assistance with needs if placed in a long-term care facility or admitted as a hospital inpatient.

Our PACE program includes:

  • Primary medical and nursing care
  • Occupational, physical and speech therapies
  • Medications and durable medical equipment
  • Laboratory and diagnostic services
  • All necessary prescription drugs
  • Skilled senior home care and personal care aides
  • Hospitalization and skilled nursing facility care
  • Care from medical specialists in cardiology, nephrology, ophthalmology, dermatology, orthopedics, surgery, podiatry and more
  • Medically necessary transportation
  • A day program with activities, meals and socialization
  • Full coordination and guidance to make sure all your health needs are met and you can get to your appointments 

Our broad range of service providers, including doctors, nurses, pharmacists, therapists and personal care aides, are readily accessible. Through these senior care services, we can manage the care of your aging loved one and help avoid the illnesses and hospitalizations that the elderly often face.

 

Program Eligibility

  • Age 55 or older
  • Live in the Franciscan Senior Health & Wellness - PACE service area
  • Have chronic health conditions that qualify for nursing home care
  • Able to live safely in the community

Respite Care & Support Groups for Caregivers

Caring for an aging loved one is one of life's most difficult and rewarding challenges. Our team of professionals will offer guidance, help and support throughout journey as a senior caregiver. With Franciscan Senior Health & Wellness, you can enjoy peace of mind, knowing that your loved one is receiving state-of-the-art medical care and other supportive senior care services.

Senior Care Day Centers

Franciscan Senior Health & Wellness provides a mixture of medical services in conjunction with elderly socialization. Participants have the option of attending our day centers that offer safe and supervised care, a nutritious lunch (dietary needs are accommodated) and group activities. Some of the activities may include:

  • Personalized exercise programs
  • Group exercise 
  • Bingo, Pokeno, Jenga, card and board games 
  • Crosswords, trivia challenges and contests 
  • Music, painting and other activities

Who Benefits from PACE

A person 55 or older who is experiencing any of the following may benefit from our program:

  • Fragmented medical care and supportive services 
  • Struggling to privately pay for all needed care 
  • Recent hospitalization due to illness or injury, resulting in decreased function 
  • Frequent falls 
  • A degenerative, long-term prognosis that the existing service delivery system has been unable to address 
  • Non-Medicaid covered needs that can be addressed by the PACE provider, whereby the result prevents a nursing facility placement (e.g. occupational therapy, physical therapy, speech therapy) 
  • Socially isolated or living alone with diagnosed dementia 
  • Missed medical appointments 
  • Involved in a day health program, but missing attendance days due to medical frailty or other circumstances 
  • Having difficulties with medication management 
  • Medically compromised with a diagnosed mental illness 
  • History of multiple emergency room visits, hospitalizations, or short-term nursing home stays 
  • Requiring extensive time commitment by case manager to ensure client health and safety 
  • Caregivers are struggling to keep up with elderly care needs 

How much does it cost to join?

PACE is funded through a combination of Medicare, Medicaid and private pay sources. There are no out-of-pocket charges for individuals who qualify for both Medicare and Medicaid.

Dual Eligible (Medicare & Medicaid)

No out of pocket cost. Medicare and Medicaid cover the cost of the program. 

Medicare eligible but not Medicaid eligible

 Participant pays for the Medicaid and Medicare Part D portion of the program (Ask our Intake Team about this amount)

Medicaid eligible*

 Medicaid pays for the program

Self-pay (neither Medicare nor Medicaid eligible)

Self-pay (neither Medicare nor Medicaid eligible)

*Medicaid Eligibility is determined by the State and participants must maintain monthly eligibility for state to continue to pay the monthly premium.
