About This Program
Franciscan Health's inpatient rehabilitation centers provide comprehensive, intensive and interdisciplinary rehabilitation to individuals following an injury, surgery or illness.
Physical, occupational, and speech therapists will partner with the patient to work to improve things such as mobility, self-care, balance, strength, coordination, fine motor, cognition, swallowing and visual skills. State of the art equipment and technology that provides resources to assist our patients who are regaining the ability to walk.
Acute inpatient rehabilitation in the hospital
To ensure our patients have the most effective treatment, we are located within the hospital, all our patients have access to specialty services (lab and radiology) and specialty physicians to meet their needs. If a person's medical condition declines, emergency services are available within the hospital.
What is the purpose of inpatient rehabilitation?
Intensive rehabilitation is meant to be short-term. The goal is to maximize independence so the patient can go home and return to the community setting as quickly as possible.
Inpatient rehabilitation acute care team
Our staff includes board-certified physiatrists who specialize in rehabilitation. Patients are also treated by certified, registered rehabilitation nurses and therapists. Each patient has access to additional specialists based upon their individual needs.
The nursing and therapy team meet regularly to discuss progress towards goals, response to treatments, barriers to progress, and discharge planning. The physician leads a formal team meeting that includes the therapists, nurses and discharge planner to discuss treatment goals, evaluate the effectiveness of treatment, determine appropriate changes to the treatment plan and discuss discharge planning.
The team works with our patients and caregivers to provide support, counseling, education and access to resources they need during and after their hospital stay.
What conditions does inpatient rehab treat?
Our inpatient rehabilitation centers treats patients who have experienced:
- Strokes
- Traumatic brain injuries
- Spinal cord injuries
- Amputations
- Multiple traumas
- Neurological diseases (e.g., Guillain-Barre Syndrome, Parkinson's disease)
- Orthopedic conditions (e.g., hip fractures or multiple major fractures)
- Cardiac conditions
- COVID-19
Who qualifies for inpatient rehabilitation?
To qualify for inpatient rehabilitation services, there must be a reasonable expectation that the patient can make significant functional progress. This is a requirement by insurance companies that we have to determine before admitting someone.
How to be admitted to inpatient rehabilitation
Franciscan Health's rehabilitation admissions team works with the patient, their family and discharge planner to help identify if inpatient rehabilitation is the appropriate next step in recovery.
A pre-admission assessment is required to be admitted to our inpatient rehabilitation program. The admissions coordinator completes this assessment, and the medical director reviews the information to determine if inpatient rehabilitation is the best setting to meet the patient's needs.
For patients in an acute-care hospital (a hospital that provides short-term care), a discharge planner should contact our center to begin the admissions process. The patient or their family may contact one of our inpatient rehabilitation centers directly as well.
What happens at a rehabilitation hospital
At an inpatient rehabilitation hospital, a patient will typically engage in at least three hours of physical, occupational and/or speech therapy per day. The goal is to intensively work on exercises and skills that will improve function and independence.
- Our patients dress in their own clothes. Comfortable attire and shoes are recommended.
- Therapy is individualized. Physical, occupational and/or speech therapy is one-on-one for at least 15 hours per week (typically three hours per day at least five days per week).
- We offer 24/7 rehabilitation nursing. Expect nurses to ask you to practice thing you learn in therapy.
- The rehabilitation physician leads your treatment plan during your rehab stay. Internal medicine or other specialists may see you daily or as needed. There is 24/7 physician availability on-site through the hospital.
- Caregiver/family involvement and training are expected. There may be things you still require some help with when it is time to leave the hospital.
- Discharge planning is ongoing from the beginning of rehabilitation. Our discharge planners will work closely with you during your stay and provide resources to help you transition back to the community.
