Overview
The inpatient rehabilitation center at Franciscan Health Lafayette provides quality rehabilitation services in a patient-centered environment. Our comprehensive compassionate team of physicians, nurses, and therapists work together with just one goal in mind - helping patients reach their highest potential toward regaining their independence. Services are delivered through patient and family focused activities to support a holistic approach through mind, body, and spirit.
Inpatient rehabilitation is designed to maximize a patient's physical function after illness or injury. We want patients to be able to perform everyday tasks to the best of their ability before going home.
Admission Process
Prior to being admitted to inpatient rehab, a thorough evaluation will be completed to determine your individual needs and if you qualify.
Once admitted, our team will work with you to develop a customized rehabilitation plan that will help you achieve the skills you need to live as independently as possible. The team, led by Dr. Pradeep Gnanapragasam, will address your medical, functional, and cognitive needs, and work with you and your caregivers on an appropriate discharge plan.

Why Choose Us
When looking for acute inpatient rehabilitative care for yourself or a loved one, it is important to understand the difference between choosing care at a program that is located within a hospital (hospital-based) versus choosing care that is offered in a rehabilitation hospital, which is separate from the general hospital.
Check out our comparison chart of hospital-based inpatient rehab vs. freestanding rehab hospital.
